Prisma Community Care is committed to providing affordable, affirming, and inclusive health and wellness services - both at our clinic and in our community. We administer quality primary care, mental health care, testing, treatment, and education, and we do it through a compassionate and holistic approach.

Our mission is to help everyone who has faced discrimination in healthcare. Our team is made up of people from all walks of life who are specially trained to care for 2SLGBTQIA+ individuals, people of color, those affected by HIV, and the uninsured and underinsured.

We know that our medical solutions work best when our clients are experiencing freedom, safety, and joy in all areas of life. To achieve holistic wellness and dignity for each of our patients, we provide navigation services, social support services, AHCCCS enrollment, and lab-assistance cost coverage to help patients achieve whole-person wellness.

We are well-versed in the systems and structures in our communities and team up with partner organizations that can offer quality additional help to our clients. Proud of our southwest roots, we plant ourselves out in our communities and connect with people where they’re at, bringing care, education, and testing to them directly.

Get Care

Over 1 Million 2SLGBTQIA+ people in the United States don't get the appropriate health care every year. Their resources are limited or they don't know how to find compatible healthcare providers. Prisma Community Care helps individuals connect with the care they need.

WELLNESS CENTER HOURS

Mon: 8am – 5pm
Tue-Thu: 8am – 5pm
Fri: 8am – 5pm

HIV/STI testing ends 1 hour before closing.
