About Our PAC (Patient Advisory Council)
Our Patient Advisory Council allows you to have an impact on the health and wellness services provided by Prisma Community Care. Our PAC will meet with staff members four times a year to provide valuable feedback and input to assist in the development and execution of Prisma Community Care services.

We are currently looking for patients who are 18 years of age and older to join our Patient Advisory Council and represent our diverse community. If you are interested in learning more about our PAC or want to sign up to be a member, please click the button below.
What You’ll Do
- Help to continually improve services.
- Collaborate with Prisma Community Care staff.
- Be a voice for our patients and community.
- Advocate for patient and community needs.
- Create lines of communication between community and staff.
- Provide constructive input and feedback on behalf of patients and the community.
Please note, PAC duties are subject to change based on program needs.
