Prisma Community Care seeks to increase access to high-quality care and support for transgender and gender non-conforming members of our community. We provide comprehensive and individualized care and support to gender-expansive people in reaching their transformative goals.
What Gender-Affirming Care Covers:
- Mental Health Services: Counseling and support groups tailored to the needs of transgender and non-binary individuals.
- Affirming Primary Care: Routine health check-ups, preventive care, and management of chronic conditions.
- Hormone Therapy: GAHT for individuals aged 19 and older.
- Surgical Referrals: Assistance with referrals for gender-affirming surgeries.
- Social Support: Resources and support for navigating social transitions, including name and gender marker changes, and housing assistance at no cost.

We assist patients in navigating services and offer support in reaching their social, legal, and physical transformative goals by providing information on local gender-affirming resources and processes, navigating agency services, and providing referrals to our resource network and community partners. We aim to eliminate barriers to care and support.
We offer assistance with the following at no cost to our patients:
- Locating gender-affirming therapists.
- Applying for legal name and gender marker changes.
- Information on local gender-affirming surgeons.
- Information on local support and social groups.
- Free clothing and accessories through the I Have a Dream Boutique.
